For each Institutional Verification Form, fax or mail the following documents to our office:

1. State Institutional Verification Form:   Complete only applicant information.
   • Do not fill in any information required to be filled in by University personnel.

2. Click Here for Release of Information Form:

   • In order for Brigham Young University to release information regarding your education license for institutional verifications, you must fill out and sign the above release of information form.  Please include this form as a cover letter when you send your institutional verification to the Advisement Center to be signed.
