These applications MUST BE TYPED!!!  By using Adobe Reader, you will be able to type directly onto the
application form, then print it to be signed and submitted in 120 MCKB.

*Please read all of the information on the first page of the application carefully.
There is important information pertaining to the application and your Student Teaching/Internship experience.*

If your are interested in applying for an Internship, you must obtain an Intern Clearace form from 120 MCKB.  You will need to get all of the required signatures and turn it in with your student teaching/internship application.

If you are planning to apply for a National / International Student Teaching Program, you must fill out one of the above student teaching/internship applications, as well as the Information Form for National/International Student Teaching(PDF).  Contact Merrell and Mary Hansen at #801-422-8117 or email them at nistp@byu.edu with any questions on these programs.
