Application Forms
Filling Out and Printing Application Forms
These applications have been created to use the form feature in Adobe Reader. This allows you to complete appropriate fields on the computer before printing. If you don't have Adobe Reader, you can download it from adobe.com. Downloading and installing this plug-in will allow you to complete and print these forms. (This is free and easy to download.)
It is important to realize that the work done on the screen can be printed but not saved.
To enter in the required information, click with the mouse where you want to begin. When the cursor appears, you can start typing. Use the tab key to move from one field to another. When an answer requires a yes, no, or similar answer, click beside your choice and a check mark will be placed in that area. As you fill in your information, Adobe Reader will add it to all of the other places where that same information is needed. For example, when you first type in your name, it will be automatically be filled in for all of the other places where name is required.
Please double check that all the information is correct before you print. If you have made a mistake, just click in that field and make the correction.
When entering in address information, use the 2 or 3 letter abbreviation for states.
Obtaining Program Application Forms
Applications for each of the education programs can be found on the Teacher Education Website under Students and then Forms and Applications.
The Technology Skills Assessment (TSA) must be completed before applying to your major program.
