Release of Information Forms

Please allow 10 business days to process your request.

State Institutional Verification:

For each Institutional Verification Form, fax or mail the following documents to our office:

1. State Institutional Verification Form:   Complete only applicant information. Do not fill in any information required to be filled in by University personnel.

2. Release of Information Form:  In order for Brigham Young university to release information regarding your teaching license for institutional verification, you must fill out and sign a release of information form.  Please include it as a cover letter when you send your State Institutional Verification Form to Education Student Services.

Student Teaching / Internship Evaluations:

To request a copy of your Student Teaching / Internship Evaluations you will need to complete a Release of Information Form.

Personal Information:

For a release of your personal information, fax or mail the following document to our office:

1. Personal Release of Information Form: In order for Brigham Young University to release personal information, such as PRAXIS scores, you must fill out and sign this form.
